CAHPS Registration Deadline: What Practices and ACOs Need to Do Before June 30
If your practice or ACO is using CAHPS as one of your four MIPS quality measures, you have a hard deadline coming up: registration must be completed by June 30.
This isn't a soft reminder; missing this deadline can disrupt your entire quality reporting strategy for the performance year. Here's what you need to know and what to do right now.
What Is CAHPS and Why Does It Matter for MIPS?
The Consumer Assessment of Healthcare Providers and Systems (CAHPS) for MIPS survey captures something that clinical quality measures can't: the patient's direct experience of care. It covers communication, care coordination, access, and overall experience -domains that matter both to CMS and to your patients.
For practices that use CAHPS to satisfy one of their four required quality measures, it carries real weight in the scoring process. But unlike most MIPS quality measures, CAHPS requires a formal registration step before you can administer it. That step has a deadline, and that deadline is June 30.
A note for ACOs: If your ACO participates in the Medicare Shared Savings Program (MSSP) and reports under the APP Plus quality measure set, you are automatically registered for CAHPS -no separate registration is required. However, you still need to contract with a CMS-approved survey vendor to administer the survey. If your ACO is participating in a MIPS Value Pathway (MVP) that includes CAHPS, separate registration by June 30 is required.
Why the June 30 Deadline Is Non-Negotiable
The registration window exists so that CMS and approved CAHPS vendors can confirm eligibility, set up administration logistics, and ensure the survey is fielded correctly during the performance year.
If your practice misses the cutoff:
You lose the option to include CAHPS in your quality measure submission for this reporting year.
You may need to scramble to identify replacement measures, potentially with fewer good options available.
Groups close to the four-measure threshold face real risk of incomplete reporting -and the payment adjustment consequences that come with it.
This is especially consequential for ACOs and larger group practices that have intentionally built CAHPS into their quality strategy. A missed registration is a structural problem, not just a paperwork inconvenience.
Who Needs to Register
You need to register if:
Your practice or group has identified CAHPS as one of your four MIPS quality measures for this performance year.
You're still finalizing your quality measure selection but CAHPS is under serious consideration.
Your ACO is participating in an MVP that includes CAHPS.
You do not need to separately register if you are an MSSP ACO reporting under APP Plus -but you still must contract with a CMS-approved vendor to administer the survey. "Automatically registered" does not mean "automatically administered."
Don't assume that a prior year's registration carries over automatically. Each performance year requires fresh confirmation, and eligibility criteria can shift. Verify that your TIN, reporting entity, and practice information are current and correctly entered before submitting.
What to Do Before June 30
1. Confirm your quality measure plan. If CAHPS is one of your four measures, registration should be your immediate priority, not something to handle in the last week of June.
2. Verify eligibility. Check that your practice or group meets the requirements to use CAHPS under your specific reporting pathway (individual, group, ACO, etc.).
3. Confirm your vendor or submission pathway. CAHPS for MIPS must be administered through a CMS-approved survey vendor. Ensure that relationship is in place before the registration deadline.
4. Review your contact and TIN information. Small errors in setup things like wrong TIN, outdated contact information, mismatched entity names, can create complications that are hard to resolve after the deadline passes.
5. Assign internal ownership. Designate a specific person responsible for completing registration and following up. CAHPS preparation tends to fall through the cracks when no one person owns it.
Common Mistakes to Avoid
Assuming CAHPS is automatic -unless you're an MSSP ACO on APP Plus. For everyone else, without registration, CAHPS cannot be counted toward your quality measures, regardless of your history with the survey. And even MSSP ACOs who skip registration still need a vendor contract -automatic registration doesn't mean the survey runs itself.
Waiting until late June. The deadline is June 30, but the process shouldn't start on June 25. Leave time to resolve eligibility questions, vendor confirmations, and data entry issues.
Assuming prior-year registration still applies. Always verify registration status for the current performance year.
Not confirming measure count impact. If CAHPS falls through and you only have three confirmed quality measures, you're looking at a reporting gap. Know your fallback before you need it.
